Yes, TW did fly IND-CVG, I beleive AL did also at some point. As far as DCA, TW and AL both had flight between the 2.

US had DCA right up to the end, I think.

American also had flights to DCA in the late 70's with 727-200s...3 daily at one point.

Yes, CVG had service from IND with AA and DL (and I believe TW at one point also). DL used DC-9s at one point, and also 727s and MD-80s.
